What do you do if your GLP-1 stops working?
Or maybe it never worked all that well in the first place?
Those are not a hypothetical questions.
While GLP-1 drugs have helped millions of people lose weight, some people barely respond, some experience debilitating side effects, and many wonder whether the benefits will eventually fade.
So today we’re going to tackle the difficult questions:
Is GLP-1 resistance real?
And if it is, what can you actually do about it?
I’m going to give it to you straight—the good, the bad, and the ugly.
And while some of the data may put a frown on your face, we will ultimately end up in a place of optimism and armed with practical tools.
GLP-1s: A Metabolic Injustice?
Let’s begin with a scenario:
Imagine two people (persons A and B) start the exact same GLP-1 medication… at the same dose… at the same starting BMI… and on the exact same day.
Six months later:
Person A has lost 60 pounds and feels fantastic.
Person B has lost 8 pounds and is nauseated all the time.
What’s the difference?
Part of the answer may be written in their DNA.
There’s certainly evidence that the effectiveness of GLP-1s varies substantially from person to person. Other medications matter. Sex matters. Age matters. And increasingly, it looks like genetics matter as well.
One high-profile paper published a few months ago in Nature reported that genetic variants in the GLP-1 receptor and the receptor for another hormone, GIP, influenced both the effectiveness and side effects of GLP-1 medications.
Some people lose a tremendous amount of weight with minimal side effects.
Others may lose less weight and experience significant side effects.
There is, to some extent, a genetic roll of the dice.
If you look at many of the clinical trials, the average reduction in body weight with semaglutide is around 10–15%. But averages can be deceiving:
Some people lose >25% of their body weight
Other people lose <5% of their weight.
I’ve even seen people gain weight while on a high-dose of GLP-1. It happens.
And let me be clear: this isn’t necessarily just a matter of some people “eating through” the medication.
That’s just blaming the victim.
While this might sound pessimistic because, short of gene editing, your genes are largely set, it’s actually profoundly optimistic.
Because once you understand the biology, solutions begin to emerge.
If you’re GLP-1 resistant, there are other levers you can pull.
Or even if genetics aren’t your problem, what happens if the GLP-1 honeymoon period starts to fade. The drug seems to stop working? What do you do then?
