Why Doctors Are Suddenly Interested in Tadalafil

Most people know tadalafil as a treatment for erectile dysfunction, though it also carries FDA approval for conditions like benign prostatic hyperplasia and pulmonary arterial hypertension.
But a growing number of clinicians and researchers are now asking a much broader question:
What if tadalafil has meaningful effects on the heart, brain, blood vessels, exercise performance, and perhaps even mortality?
That question becomes easier to understand once you examine what tadalafil actually does.

How Tadalafil Actually Works: PDE5 Inhibition
Tadalafil belongs to a class of medications known as PDE5 inhibitors.
To understand why that matters, we first need to understand how blood vessels relax and expand.
The smooth muscle surrounding arteries controls how constricted or dilated those vessels are.
When smooth muscle relaxes, arteries widen, blood flow improves, and tissues receive more oxygen and nutrients.
One of the key signals responsible for this relaxation is a molecule called cyclic GMP, or cGMP.
In simple terms: cGMP → more smooth muscle relaxation → better blood flow.
Now, you need to know what causes cGMP to rise and fall.
Rise: Nitric oxide stimulates the production of cGMP through an enzyme called soluble guanylate cyclase.
Fall: Then there’s another enzyme, PDE5, that breaks cGMP down, effectively shutting off the signal.
This is where tadalafil enters the picture.
By inhibiting PDE5, tadalafil prevents the destruction of cGMP. The result is sustained smooth muscle relaxation, expanded blood vessels, and improved circulation.
Importantly, PDE5 is found throughout the body, including within blood vessels in your muscles, heart, and brain.
With this mechanism and wide enzyme (PDE5) distribution, it’s easier to see why tadalafil might have such a broad range of health effects.

Let’s begin with tadalafil for cardiovascular health.
Tadalafil and Cardiovascular Health
Here, the literature is actually rather impressive—though admittedly, the strongest controlled human trial data come from men who already had some form of underlying cardiometabolic dysfunction.
For example, a 2005 study conducted in 32 men with elevated cardiovascular risk found that tadalafil improved a measure of blood vessel function, flow-mediated dilation, while also lowering levels of endothelin-1, a molecule associated with vascular dysfunction and constriction.
In this study, 32 patients were prescribed either tadalafil at 20 mg every other day or a placebo control.
Impressively, tadalafil more than doubled flow-mediated dilation.
That is no small effect.
Because flow-mediated dilation is not some obscure laboratory number, it is a clear functional measure of vascular health and endothelial performance.
